Boudoir' / Glamour Client
Release and Confidentiality Statement
In consideration of my engagement as a client of Danmar Imaging for a 'Phantasy Fotos - Boudoir' and Glamour' photo shoot, hereafter referred to as The Photographer, on the terms and fees here in after stated (sitting fee and proof fee of $ _____.___ ), I DO NOT give the photographer, his legal representatives and/or those acting with his permission, the right to use, reuse and or publish photographic images, pictures or portraits of me, or in which I may be distorted in character, or in form, in conjunction with my own or a fictitious name, through any media by the photographer at his studio or elsewhere, for any purpose whatsoever.
ALL PHOTOS ARE TO BE USED FOR MY PERSONAL USE ONLY.

Hereby will not discharge photographs covered by this restricted
release and taken by The Photographer, his representatives, employees
or any person or persons, to any corporation or corporations
or other entity for whom he might be acting. The photographs
are NOT TO BE SOLD or DISTRIBUTED by me as a finished product,
in whole or in part. I will not blur or alter, or use in composite
form, either intentionally or otherwise, or in any type of publication
without the approval of a valid seperate photographers release.
I understand that my Personal Boudoir' photoshoot images are
for my personal use only , and may not be published in
any form, other than for my personal use (enlargements and other
personal prints, used on my personal computer as a wallpaper,
screen savers, personal photo album, etc.). Any commercial
use requires a seperate photographers release prior
to there use in any medium. All images remain the copyright of
the photographer unless a seperate release transfers ownership
to me. If images are published in any form (including
but not limited to books, magazines, photo post cards, any commercial
or pay web site, or as any part of a for-pay item or service),
I will be billed $1000 per day of use for each image used without
a vaild photographers release. This aggreement is binding
on me, my heirs, any legal representatives and all others acting
in my behalf.
I, _______________________________________________________________ (client/model) DO NOT give The Photographer or his representatives the right to use any proof, negative, slide or finished product similar to a photograph as a sample or for display purposes to show other potential clients. THE ONLY PERSONS AUTHORIZED TO REVIEW THE PROOFS ARE MYSELF AND _____________________________________________________________ .
I have read the forgoing client release & confidentiality statement, authorization and agreement before affixing my signature below, and warrant that I fully understand the contents thereof and desire Danmar Imaging to photograph me as stated for my personal use. I warrant that I am an adult over 18 years of age, and legally able to contract in my own behalf.
